An authenticator app makes the six-digit codes that protect your email, bank and social media accounts. If you lose access to it, you can be locked out of everything at once, so it matters who holds your codes and their backup. The big US apps tie that backup to your Google or Microsoft account. European authenticators let you back up your codes encrypted, or keep them on a key in your pocket.
